Are you a patient or carer of someone with two or more long term health conditions who needs regular support from health and social care services to stay well?
If so, we would like to invite you to be part of a small focus group to talk about your experiences. We will also let you know about the way Health and Social Care staff are going to work differently to avoid duplication (patients having to explain their circumstances to different people all the time), and by providing better coordinated and personalised care that really meets the needs of individuals and their families.
We will do this by getting teams of health and care professionals (nurses, social workers, GPs, support organisations etc) to work together so that they are all aware of the needs of the patient rather than just the bit they are responsible for. So, for example, each professional would know that the patient receives visits from the community nursing service, has carers in twice a day and receives meals on wheels. Your GP will be the key coordinator for this way of working.
